Applications for Education Benefits
All Applications for Education Benefits & Confidentiality Waivers must be submitted annually. Forms for the upcoming/current school year reset each & must be submitted after July 1.
If your student hasn't already directly qualified for Educational Benefits (based on income, SNAP, or Medicaid),
1. Complete Application for Education Benefits:

b. Submit Paper Version of: 26-27 Educational Benefits Application to:
In addition, your consent in order for our department to share this information which may enable reduced fees for:
- Athletics
- iPad/Laptop
- Instrument Rental
2. Submit Program Shared Consent Form (Confidentiality Waiver)
- Please note: One form submission for each of your students in your family/household must be completed separately!
